Counselor II - ACT/WCD

The Counselor II is a valuable member of our (ACT) Assertive Community Treatment Team to provide intensive case management for adults with chronic, severe mental illness. Our team works to reduce hospitalizations, avoid nursing home placement, ensure basic needs are met, maintain engagement in treatment, and promote harm reduction. Responsibilities include evaluating clients' plans and implementing treatment plans with clients on assigned caseloads. Conducts individual, group, and family counseling, case management, outreach, education, monitoring, referral, linkage, follow-ups, and documentation. Schedule: 37.5 hours a week Monday - Friday (8:30 am to 4:30 pm) Maintains regular contact with an active caseload- most of which is conducted in clients’ homes and in the community. Provides direct services to clients including individual and group treatment. Consults with the supervisor regarding transfer/placement of a client to another level of service, as needed. Facilitates with clients and collaborates with the treatment team to develop, review and update treatment goals and referrals both internally and externally.
