The Care Coordinator facilitates care coordination for youth enrolled in the Illinois Department of Healthcare and Family Services (HFS) Pathways to Success program. Youth are enrolled in either the High-Fidelity Wraparound Services or Intensive Care Coordination Services. The Care Coordinator will utilize the Principles of Wraparound and System of Care when providing services to families. This position is home-based, meaning there is not an assigned office, but the person is expected to travel within Lake County to attend meetings with youth and their families. Scheduled Hours: 37.5 hours a week Monday - Friday 2 days (9:00am-5:00pm) 3 days evening shift (flexible hours) Provide direct services to youth and families while managing a caseload of either 10-12 clients for High-Fidelity Wraparound or 16-18 clients for Intensive Care Coordination. Coordinate and monitor delivery of Individual Support Services (ISS) and Therapeutic Support Services (TSS) for youth. Provide referrals to additional Pathways to Success services, such as Therapeutic Mentoring, Intensive Home-Based Services, Family Peer Support, and Respite.
