About the role: Care Coordinators serve as the central entry point and operational backbone for the Care Management department. They provide comprehensive care coordination for members, including those in Complex Care Management (CCM), Readmission Prevention (RAP), and those who do not fit into existing highly specialized programming. By taking ownership of administrative tasks, managing post-discharge transportation needs, and resolving inbound inquiries with a focus on one-call resolution, the Care Coordination Team allows our RN Care Manager
