Ryan White Medical Case Manager

Under the supervision of the Ryan White Case Management Coordinator, the Medical Case Manager coordinates the medical care of clients living with HIV/AIDS, in conjunction with the Medical Management Clinic at LCHD/CHC and works with clients to develop a supportive relationship while facilitating access and use of available services. Work involves direct client contact and is performed in accordance with state, local agency and grant policies and procedures. Scheduled Hours: 37.5 hours per week Provides direct services to Ryan White clients in community, clinic and home settings according to need and established program protocols and procedures. Facilitates, organizes, and utilizes systems for tracking Ryan White client services and demographic information Completes and submits client intakes, eligibility assessments, ADAP applications, and HIV-related medical expenses in a timely manner into Provide database.
